Thank you for choosing Kentwood Family Physicians as your healthcare provider! Please observe the following guidelines when making your first visit:
- Please complete our new patient forms and bring them with you to your appointment. If you have not already received these forms, they are available for download (see below).
- Please bring your insurance card(s) with you.
- If the patient is a minor, a parent or legal guardian must accompany him or her to the initial office visit.
- Please arrive 15 minutes prior to your appointment to complete registration process.
